_I certify that my dog(s) are in good health, have not been ill with any communicable diseases or parasites in the 30 days, has/have not harmed or shown aggressive or threatening behavior towards any person or and other dog(s). _All dogs participating at Kara’s K9 Retreat must be current and show proof on their vaccinations. Required vaccinations are: Rabies (1 or 3 year)  Distemper/Parvo (1 or 3 year) Bordetella (every 6 months)  

 

_I understand and agree that my dog(s)  are required to be spayed/neutered by the age of 7 months. 

 

_I understand that during normal dog play, my dog(s) may sustain injuries.  All dog play is constantly supervised to avoid injury but still may occur despite the best supervision.  This includes but not limited to:

1. Injuries, usually minor, such as broken nails, sore pads, sprains, scratches, puncture wounds, abrasions, cuts, etc. 

2. Communicable illnesses and/or viruses such as Kennel Cough, Canine Influenza, pink eye and/or upper respiratory infections can be transferred among dogs.  This is similar to children in daycare where contagious viruses spread.  I further understand that even if my dog(s) has/have been given the Bordetella vaccine, there is a chance that my dog (s) can still contract kennel Cough.  I agree that I will NOT hold Kara’s K9 Retreat responsible if my dog(s) contract Kennel Cough or any other illness/diseases. 

3. Spa services, such as bathing, may carry a risk of skin and/or eye irritations and nicks causing bleeding during nail trims.

 

_I understand that should my dog(s) be observed with fleas and/or ticks while attending Kara’s K9 Retreat, a flea bath will be given and charged to the owner at the bath rate based on size and coat. 

 

_I agree to maintain my dog(s) on regular flea/tick preventative while attending Kara’s K9 Retreat.  

 

_I understand that if any health and/or behavioral problems develop with my dog(s) that Kara’s K9 Retreat will not be held liable for any said issues.  Furthermore, if health and/or behavioral problems develop with my dog(s) they will be treated as deemed best by the staff at Kara’s K9 Retreat within their sole discretion, and that I assume full financial responsibility for any and all expenses involved. 

 

_I understand that in the event that my dog(s) appears to be ill or experiencing a medical problem, Kara’s K9 Retreat will attempt to contact me for instructions prior to seeking veterinary care.  If I or the emergency contact cannot be reach or if a medical emergency arises, I agree that Kara’s K9 Retreat may use its reasonable discretion in seeking veterinary care on my behalf and I will be responsible for all medical bills, boarding and/or  

 medication cost.  I hereby grant Kara’s K9 Retreat the authorization to treat my pet as  the    attending veterinary deems necessary.  Medical fees are to be paid in full to Kara’s K9 Retreat at the time of pick up. 

 

_I understand that transportation for my dog(s) may be required. I acknowledge that allowing Kara’s K9 Retreat, its officers, directors, members, employees, or agents to transport my dog(s) involves risks of injury during transport. I knowingly accept and assume all known and unknown risks in allowing my dog(s) to be transported. 

 

_I agree to hold Kara’s K9 Retreat, its owners, employees or agents harmless from any claim (including legal fees) for loss of pet(s) by disappearance, theft, death, fire, or otherwise, and from any claim, damage or injury to the dog(s) whether such loss be, disappearance, theft, damage or injury caused or alleged to be caused by negligence of Kara’s K9 Retreat or any parties aforementioned. 

 

_I understand that Kara’s K9 Retreat closes at 6:00pm.  It is hereby acknowledged that should I fail to pick up my dog(s) by closing time, a late fee will be charged $1 per minute after 6 pm. All dogs not picked up by 6:15 pm will be assumed boarding overnight.  A $15 late fee and boarding charges will charged at the time of pick up. 

 

_I agree that pictures/videos of my dog(s) may be used by Kara’s K9 Retreat staff for advertising and promotional materials and social media. 

 

_I understand and agree that Kara’s K9 Retreat is not held responsible for any damaged, broken or lost belongings brought into the facility. 

 

_I understand that the attendance of dog daycare is not without risk to myself, members of my family, guest(s), my dog(s) and/or other dogs, because some of the dogs to which I will be exposed to may be difficult to control and may the cause of injury even when handled with the greatest amount of care. I hereby release, indemnify and forever hold harmless Kara’s K9 Retreat, its owner, employees, officers, agents, customers, visitors and guest from any and all liability of any nature, for injury or damage that I or my dog(s) or members of my family or my guests may suffer. This includes specifically, but not limited to, any injury or damage resulting from the action or inaction of any dog or its owner or handler, Kara’s K9 Retreat, our employees, owners, or agents. I expressly assume the risk, negligence or any damage or injury while attending dog daycare or while on the grounds or the surrounding area thereto.
